What To Do Next
If you would like to meet the surgeons and learn more about our Program and the weight loss options available to you, please attend one of our monthly Information Sessions or follow the steps below to schedule a surgical consultation.
- Fill out the attached Insurance Information Form and mail or fax it to our office. If you prefer to call, we will take your information over the phone. We will call you in 2-3 days when we have received verification of benefits. If bariatric coverage is not available we will discuss the financing options available. (attachment)
- Once coverage has been determined, we will schedule your surgical consultation with one of our Bariatric surgeons.
- Please print out and complete the New Patient Information form and the Medical & Dietary History Questionnaire. Bring the completed forms to your first appointment. (attachment)
- After your surgical consultation, a case manager will discuss with you in detail the necessary steps you will have to take from this point forward. At a minimum you will need to see our psychologist and nutritionist, although most insurance companies require not only a psychological consultation but 6 months of a medically supervised diet as well.
