Print out the ten pages in order and staple. Complete all information, and bring with you to orientation.
**** Page 7 “Attestation of Background” is to be signed by your school representative and inserted in this packet.
Hospital ID/Access Badges – Because our badges allow for certain area accesses, we will obtain a check deposit of $25. The deposit will be returned when the badge is returned to Sue Stanish in the Education Department on the last day of the clinical rotation.

2013 Seasonal Influenza Vaccine Documentation
All students, residents and interns must submit proof of flu vaccine or sign the Declination form for seasonal influenza.

Please complete an Orientation by following the instructions for the Student-Resident Orientation. Please read and follow the instructions for the Instructor Access to Administer Medication and Meditech. Then complete the very first block of the first page of the 2013 Nursing instructor EFD Security Access Form and return to Sue Stanish via email susan.stanish@hcahealthcare.com. Please submit your BCMA and Meditech class dates as well but remember that the EFD Security Access Form needs to be received at least two weeks prior to your class dates.
